Per California Education Code (EC) Section 33190 , anyone offering or conducting private school instruction at the elementary or high school level shall annually file a Private School Affidavit with the California Department of Education (CDE). The affidavit shall be completed by full-time private schools meeting the following criteria:

Prior to starting the affidavit filing process, please ensure that you have all of the required information and have read the Instructions for Filing a Private School Affidavit.
To complete the affidavit, use the Next and Previous buttons to navigate through each section of the affidavit. When complete, select the Review button to confirm all affidavit fields are accurate. To make changes while viewing the Summary page, select the Edit link located in each section. Otherwise select the Submit Affidavit button to electronically file the affidavit with the CDE. Once filed, you will be redirected to a confirmation page and an automated e-mail confirmation will be sent to the owner and the email address provided for the school.
To begin the affidavit filing process, indicate whether or not the school is a full-time private school and meets the exemptions as set forth in EC Section 48222 .
